Hyperbarisk oxygenterapi er den medicinske brug af oxygen ved højere niveauer end atmosfærisk tryk. Typisk vil en hyperbarisk oxygenbehandling bestå af en længere række af behandlinger på 15-90 minutters varighed, hvor ilt under tryk bruges i behandling af forskellige lidelser.
Trykkammerbehandling anvendes til behandling af bl.a. iskæmiske sår, strålefølger og visse infektioner. Nu tyder det på, at det måske også kan hjælpe syge covid-patienter.
Behandlingen henvender sig til indlagte risikopatienter.
I et trykkammer er der et tryk, som svarer til at patienten er 14 meter under havets overflade, og når patienten her får iltbehandling, optages 2,5 gange mere ilt af kroppen gennem lungevævet, hvilket reducerer betændelse.
Der skal 200 personer med i forsøget, som vil blive foretaget i samarbejde med forskere i Tyskland og Rumænien. 100 personer i forsøget skal have trykkammerbehandlingen.
“Hvis forsøget går så godt, som vi håber, kan mindst 20 undgå intensiv. Vi måler dog også andre ting, og der er enorme fordele for både patienter og hospitaler, også selv om metoden ikke er så godt, som vi håbe,” siger han.

In short, the study aims to see if treatment with pure oxygen in a pressurized chamber equivalent to 14 meters of pressure underwater could reduce inflammation in the lungs of 19 patients, and hence the need for one IVA admission and one Ventilator. .
Göran describes how he immediately noticed a difference. He said to lie down in the hyperbaric chamber and stay there for an hour, with a five-minute break after 30 minutes. The reason is that exposure to 100 percent oxygen for too long can be risky.
– When I was in this house it was like going to heaven. It was an absolutely fantastic experience and I was able to speak as always.
Göran was treated five times in seven days. When he speaks to Expressen on Friday, he says he’ll likely be released from the hospital on Saturday.
It was a total of two weeks at Karolinska University Hospital.
– Everything just turned completely around. I got a lot better quickly and after a while I was able to switch to the first department, which is where I started. And they gradually went back to the oxygen and now I don’t need any oxygen at all, he says.
– Now I can celebrate Lucia at home instead of Christmas at IVA. It feels pretty good.

The doctor: “Incredibly enjoyable”
Göran carefully points out that the treatment was part of a clinical trial and that it has not yet been proven to have helped him.
– It may still have been a coincidence, even if I don’t think so. However, this will only be known after the study has been completed.
– But I feel incredibly privileged to have received first-class support from Karolinska. Everyone here does a fantastic job and the staff work like animals and work very long shifts.
Anders Kjellberg, the doctor who leads the study, sees the test as positive. That summer, two patients had to test the treatment in Karlskrona, but this was the first time he was allowed to do it.
– It is incredibly gratifying that I saw this with my own eyes. This case means that I now know that we at Karolinska can do this study in a good and safe way, he says.
– Now we have exactly two patients who have signed up for the study. I’ll talk to them again on Saturday and see if they meet all the criteria.
For the study, the doctor is looking for people who are moderately ill with Covid-19 and who are hospitalized with oxygen. You must also have at least two risk factors for needing IVA care. However, you must not be pregnant or have severe carbon monoxide as the treatment can damage the lungs.
Those who pass and choose to join are then randomly divided into two groups. One is treated in the hyperbaric chamber while the other becomes a control group to evaluate whether the treatment is working.
– We hope to be able to show that with this treatment we can actually prevent inclusion in IVA and that those who are admitted do better so that fewer people die, says Anders Kjellberg.
The treatment had previously been tested in studies in the United States, among others, and there patients followed the same pattern as the first in Stockholm. They showed an improvement after the first treatment, after the third a significant improvement – and after the fifth several were able to leave the hospital.
The study in brief
The study evaluates whether Covid patients react positively to treatment with oxygen in the pressure chamber. The oxygen corresponds to a pressure of 14 meters under water.
That’s how it works
For some of those who are seriously ill with Covid-19, part of the innate immune system runs amok. The treatment signals the part of the immune system to stop attacking, resulting in decreased inflammation in the patient.
That is the goal
Doctors hope to show that treatment can prevent IVA entry and that those who are admitted are better off.

Here are the criteria that apply to the study
INCLUSION CRITERIA:
Adult patients between 18 and 90 years of age
PFI <26.7 kPa (~ 6 L O2 to maintain saturation above 90%)
Suspected or verified SARS-CoV-2 infection
At least two risk factors for increased morbidity / mortality
Defined risk factors in the study:
Age over 50 years

Cardiovascular disease

Autoimmune disease
EXCLUSION CRITERIA:
ARDS / pneumonia for any reason other than COVID-19
Known pregnancy or positive pregnancy test in women of childbearing age
Patients with known pulmonary fibrosis greater than 10% were verified with DT
DT- or spirometry-verified severe COPD with emphysema
Probably not required IVA approval within 7 days (spontaneous improvement)
Contraindication to HBO or not possible / risk for transportation to HBO according to local routines
